How do I schedule an appointment?

You can schedule an appointment in three ways:

  1. Online: Use our online booking system on our website (available 24/7)
  2. Phone: Call us at (123) 456-7890 during business hours (Monday-Friday 8am-6pm, Saturday 9am-2pm)
  3. In-person: Visit our clinic during operating hours

New patients should arrive 15 minutes early to complete registration forms.

What is your cancellation policy?

We require at least 24 hours' notice for appointment cancellations or rescheduling. This allows us to offer the slot to other patients. Late cancellations or no-shows may incur a fee:

  • Routine exams: $50 fee
  • Specialty consultations: $75 fee
  • Surgical procedures: $100 fee

We understand emergencies happen - please call us as soon as possible if you need to cancel.

What should I bring to my appointment?

Please bring the following to your appointment:

  • Photo ID (driver's license, passport, etc.)
  • Current insurance card(s)
  • List of current medications and supplements
  • Current eyeglasses and/or contact lenses
  • Previous eye exam records (if available)
  • Referral form (if required by your insurance)
  • Payment method for any co-pays or deductibles
How long does a typical appointment last?

Appointment duration varies based on the type of visit:

  • Comprehensive eye exam: 60-90 minutes
  • Routine check-up: 30-45 minutes
  • Contact lens fitting: 45-60 minutes
  • Specialty consultation: 60-120 minutes
  • Follow-up visit: 15-30 minutes

Please plan to arrive 15 minutes before your scheduled appointment time.

What to expect during your examination

How often should I get an eye exam?

The recommended frequency depends on age, risk factors, and existing conditions:

  • Children (under 18): Annually, or as recommended by your eye doctor
  • Adults (18-60): Every 2 years if no vision problems exist
  • Adults (61+): Annually
  • Contact lens wearers: Annually
  • Those with diabetes: Annually
  • Those with family history of eye disease: Annually or as recommended

Your eye doctor will recommend a personalized schedule based on your eye health.

What tests are included in a comprehensive eye exam?

A comprehensive eye exam typically includes:

  • Visual acuity test: Measures how well you see at various distances
  • Refraction assessment: Determines your exact eyeglass prescription
  • Eye movement and alignment testing: Checks how well your eyes work together
  • Visual field test: Checks your peripheral (side) vision
  • Slit-lamp examination: Microscopic examination of the front parts of your eyes
  • Tonometry: Measures eye pressure to screen for glaucoma
  • Retinal examination: Examination of the back of your eyes (retina, optic nerve, blood vessels)
  • Optional tests: Based on age, symptoms, or risk factors
Do I need to dilate my eyes for the exam?

Dilation is typically recommended for comprehensive eye exams because it allows for a better view of the retina and optic nerve. However, we also offer non-dilated retinal imaging using advanced technology. Your doctor will recommend the best option based on:

  • Your age and medical history
  • Specific symptoms or concerns
  • Risk factors for eye diseases
  • Quality of view needed for accurate diagnosis

If your eyes are dilated, you may experience light sensitivity and blurred vision for 4-6 hours. We recommend bringing sunglasses and arranging transportation if needed.

What insurance plans do you accept?

We accept most major insurance plans, including:

  • Medical Insurance: Aetna, Blue Cross Blue Shield, Cigna, UnitedHealthcare, Humana, Medicare, Medicaid
  • Vision Insurance: VSP, EyeMed, Davis Vision, Spectera

What if I don't have vision insurance?

We offer several options for patients without vision insurance:

  • Self-pay rates: Competitive pricing for exams and services
  • ClearVision Membership Plan: Annual membership includes comprehensive exam, discounts on eyewear, and other benefits
  • Flexible Spending Accounts (FSA) / Health Savings Accounts (HSA): We accept payment from these tax-advantaged accounts
  • Payment plans: Available for larger balances (subject to approval)

When is payment expected?

Payment is expected at the time of service unless other arrangements have been made. This includes:

  • Co-pays and deductibles (as determined by your insurance)
  • Any services not covered by insurance
  • Self-pay patients: Full payment for services rendered
  • Eyewear and contact lenses: Payment in full when ordered

We accept cash, checks, and all major credit cards. For your convenience, we also accept HSA and FSA cards. Estimates for services can be provided upon request.

What are your hours of operation?

Our regular business hours are:

  • Monday - Thursday: 8:00 AM - 6:00 PM
  • Friday: 8:00 AM - 5:00 PM
  • Saturday: 9:00 AM - 2:00 PM (by appointment only)
  • Sunday: Closed

We offer extended hours for emergencies. For after-hours emergencies, call (123) 456-7890 and follow the prompts to reach our on-call doctor.

Holiday hours may vary - check our website or call ahead during holiday seasons.

Do you offer emergency eye care?

Yes, we provide emergency eye care for urgent situations such as:

  • Eye injuries or trauma

  • Foreign objects in the eye
  • Chemical burns or exposures
  • Sudden onset of floaters or flashes of light

How do I get a copy of my medical records?

You can request your medical records in several ways:

  2. In person: Visit our clinic and complete a records release form
  3. By mail/fax: Send a written request with your signature to our medical records department
